Attached is the form that you need to fill out for your request. Let me know if you have any questions about it, I might be able to help. It then needs to be mailed to the city records office along with a $5.00 cheque (I know... ). If you want to fill it out, sign, and forward an e-version to me, I can print it here and mail it out—it'll be faster. Hi Camille, 

I've started a spreadsheet for the timeline. The section has a 250-word limit. At first, I thought 250 would be restrictive, however, the spreadsheet only has 63 words currently. Realizing that 250 is quite generous for a timeline, I think it would be good to be fairly detailed: https://docs.google.com/spreadsheets/#redacted

FYI - I know the CCA portal doesn't use a table, but this was just a better format for me to organize my thoughts.

Hi again, Camille,

On the checklist, I notice that you have captioning listed. I was going to put this info in the checklist document but I'm sending it here instead because I want to keep that document super simple so we can look at it and easily know what we still need!

Closed captioning can be offered and will not cost us anything. If we want to budget for ASL interpretation we should be aware that it can become a heavy budget line quickly. 

The last time I arranged for an ASL interpreter the organization that provided the service required that 2 interpreters be present at all events, regardless of their scale. I don't know if that's standard practice or unique to this organization. The rates were as follows:

Day rate for up to 8 hours - $800.00 per interpreter
Half day rate for 0.5 hrs up to 4 hours - $400 per interpreter
Preparation time, mileage, and administrative fees all additionally charged
